The site has since then been updated. It is actually three hole punched "loose leaf". The textbook copy came with the required code for the online Total Access.

When I recieved the book, it was in great condition, but the code had already been taken out. Since I am required to have access to the online materials for the course, I now need to find another way to gain access, or return this and buy another book, hoping the next one actually has the access code included.
